1. SIMPLIFIED UNDERWRITING – Up to $3000 Sickness and Accident Monthly Benefits up to age 70. No medical exams, blood work, tax returns or other underwriting tests unless required because of medical history.

• Your “OWN OCCUPATION” definition of Disability.
  Total disability means you are unable to perform the substantial and material duties of your occupation for the first five years and are receiving regular medical care by a physician. Total disability benefits will continue after five years if you are working in an occupation outside of the dental profession. If after five years you are working in the dental profession, but you have at least 20% income loss, we will continue paying you a proportionate benefit equal to your income loss. (Residual Disability Benefit).

   

• Partial (Residual), Disability Benefits payable after a 20% income loss.
  Residual Disability means you are unable to perform one or more of the substantial and material duties of your occupation, or for as much time as is normally required, and you suffer a 20% or greater income loss.

   

• Plan features and definitions will remain the same for the lifetime of the policy.

   

• Step rate (pay–as–you–go) premiums.

• Choice of waiting periods: 30, 60, 90 and 180 days.

   

• Optional Benefit Riders – Guaranteed Purchase Option, Cost of Living Benefit.
